CLINICAL EXPERIENCE

For the last three years of my basic surgical training, and the first two years of my advanced surgical training, I gained experience in both elective and emergency general surgical procedures in both the District General and Teaching Hospital setting around the state of Queensland. During this time I gained the necessary technical and management experience for a career in general surgery. This included exposure to upper GI/HPB, colorectal, vascular, breast, endocrine, cardio-thoracic and emergency surgery. I became competent in surgical endoscopy of both the upper and lower GI tract.

The following twenty-four months of this advanced surgical training were spent at the Royal Brisbane Hospital in Brisbane, Australia. This added breadth to my surgical exposure to sub specialised surgical units, and their patients with increasing complex clinical problems. I also developed competence in advanced laparoscopic surgery.

Toward the end of my period of general surgical training, I became increasingly interested in upper gastrointestinal, hepatobiliary and pancreatic surgery and therefore elected to pursue this interest during my post fellowship training.

During my period of HPB specialist training, I worked with 13 HPB surgeons from 4 countries in 4 specialist units. This has given me a comprehensive and varied exposure to all of the procedures necessary to function independently as a consultant upper GI/HPB surgeon, with the exception of liver transplantation. I have also participated actively in the research activities of each of these units and continue to pursue my own research interests, as outlined below.

Impact of bariatric surgery of glycaemic control in type II diabetes.
A retrospective analysis of 110 diabetic patients post bariatric surgery.
Resolution of diabetes in 67% of patients following laparoscopic banding, and 97% following gastric bypass at 2 years post op.

Avoid the tangle. A useful tool to hold the sutures.
During biliary anastomosis the multiple interrupted sutures can easily become entangled. This can be limited with the use of a scrubbing brush to hold the sutures. It is a, readily available, non-traumatic, instrument and been found to be invaluable aid during difficult anastomosis.
